This thread concerns the development of a form that a particular dentist
(in a dental forum) wants to use for his patients who are prescribed
treatment with an NTI. Most of the discussion is in previous posts to
the thread.

In my mind, certain questions remain:

1. Is it, the draft, a consent (to treatment) form or a fee agreement?

2. Is it an instruction sheet and a fee agreement?

3. Who is responsible for standard of care issues when prescribing NTI
for a particular diagnosis? Is it reasonable to expect (all) patients
to turn to a website to read a "form" that informs them as to the use of
this device? Or should the dentist be required to make such information
available in an office handout (of sorts) for each and every NTI
prescribed patient?
