Author 14 months PVP check-up

They finally got around to seeing me for my 12 month checkup, 2.5 months
late. I'm one of the last ones in the Laserscope PVP study using the 80 W
"green light" laser. I have felt good for the past six months and long
before that. I would say about two weeks after the PVP I was in good shape.
I'm 74, 73 when the procedure was done. Prior to that I had bhp for about
15 years. Did nothing put wait for some 3 or 4 years and then started
taking cardura, doxazosin the generic, two mg/day for 4 years, then four
mg/day for about 7 years. They then switched me to flomax, tamsulosin the
generic. The cardura worked, in that I didn't have to get up but once a
night at worst, but I have low blood pressure and it was making it a lot
lower, to the point where I had to be very careful getting up from chairs,
or bending over where my head was lower than my heart, so they changed me to
flomax. It worked but the prostate kept growing and finally got to the
point where I was ready for some relief. I decided on having the PVP, and
lucked into laser study with Dr. Nseyo as the urologist. I was warned by
him that because of so much of the median lobe being into the neck of the
bladder that I might end up with retro ejaculation, which I did and still
have.

They also tried proscar, finasteride, to reduce the size or at least stop
the growth of my prostate. It didn't. I had tried saw palmetto and that
didn't work. Found out that the mechanism both of them use was identical,
the prevention of the formation of dihydoxi testosterone, DHT, from
testosterone. DHT formation is supposed to be the reason for the benign
growth of the prostate. If one doesn't work the other won't since they both
use the method to stop prostate growth.

At the time of the operation, my prostate was 80 gms. today it measured 45
gms. Seven months ago it was 55 gms. I asked Dr Nseyo what caused the
decrease, he couldn't explain it. Said they hope for no increase in size
from after the PVP for at least a year and then just very small growth for
at least 3 years. My guess is the machine they use has at least that margin
of error. Before the operation they sized it at 65 gms and when they saw it
during the operation they said it was 80 to 85 gms. Another person, they
told him is was 90 or so gms and when they saw it at the start of the PVP
they said it was 200 gms and aborted the procedure.

My numbers before the PVP and today are: Flow rate, max 5.6 ml/sec, 21
ml/sec; Avg flow rate 2.6 ml/sec, 13 ml/sec; Volume 125 ml, 329 ml;
Retention in bladder 310 ml, 105 ml. I was surprised at the retention rate
today, almost 25%. Prior post PVP retention tests after 3 months were less
than 10%. I get up about 1 night in 3 to urinate about 4 am, otherwise I go
when I get up around 7.

I'm more than satisfied with the results, I guess I would rather not have
the retro, but the orgasms are about the same as before the PVP and my guess
is the quantity of ejaculate is about the same. My first urination after
ejaculating is almost opaque and very cloudy.

Do you know what the max size is? And then what can be done, TURP only, or
are there other options.

Jack, thanks for the report. Can you remember the progress of your volume
increase from 125 to 329 ml? Thanks, Jim W.
