uring the course of disease (such as cytokines or viral load). Projects m=
ay identify, characterize, and validate targets for ongoing or future dru=
g discovery, prognostic marker utility, or molecular epidemiology efforts=
for established disease or disease prevention. Novel agents could includ=
e, but are not restricted to, small molecules, antivirals, gene therapy, =

Fund Title:
Structural Interventions, Alcohol Use, and Risk of HIV/AIDS=20
Fund Category:
HIV/AIDS=20
Description:
This RFA requests research on the effectiveness of structural interventio=
ns that reduce the risk of HIV/AIDS transmission by changing the environm=
ent of alcohol use. Although a variety of structural and environmental in=
terventions have been employed successfully to reduce other drinking-rela=
ted problems, there has been little research that extends such efforts in=
to the realm of HIV/AIDS risk reduction. While this RFA is primarily dire=
cted toward reducing alcohol-related risks, structural interventions may =
also be developed to improve secondary prevention among individuals who a=
re already infected with HIV. These interventions may focus on increasing=
screening and identification of HIV+ alcoholics (often a hard to reach p=
opulation), engagement and retention of this difficult-to-treat and often=
non-adherent group in AIDS treatment protocols, or developing new techno=
logies to improve alcohol and HIV/AIDS treatment among these=20
multiply-diagnosed persons to address medically significant end points, s=
uch as liver failure. Importantly, structural alterations may address the=
stigma endured by HIV-positive persons with drinking problems, particula=
rly the manifold ways that felt stigma may hinder the retention of such p=
ersons in alcohol and HIV/AIDS treatment programs.=20

Description:
This initiative will stimulate research to elucidate the mechanisms, risk=
s, and benefits of using complementary and alternative medicine (CAM) in =
the management of HIV/AIDS at institutions with extensive histories of re=
search on HIV/AIDS. NCCAM is especially interested in supporting well-des=
igned studies to identify potential roles for the safe and effective use =
of CAM in the management of HIV/AIDS and its complications, to ameliorate=
medication side effects, or improve quality of life. The objectives of t=
his solicitation are to (1) identify potential roles for CAM therapies in=
managing HIV/AIDS and/or its complications: (a) prior to initiation of H=
AART; (b) in association with HAART; (c) by ameliorating HAART side effec=
ts; or (d) through increasing the quality of life for people with HIV/AID=

potential for contributing to or interfering with standard treatments=20
for HIV/AIDS and related conditions; and (3) stimulate CAM research at le=
