Have they biopsied the antrum of the stomach for H pylori during your EGD
(surely they have...)? Alternatively, a urease breath test for that bug is
quick and simple.
There are pitfalls to that testing - the sensitivity of breath testing isn't
that high, likewise urease testing on biopsied specimens (CLO test, etc) can
be inaccurate. Perhaps more accurate is H&E staining of biopsied specimens
to look for the actual organisms. Additionally, in cases of treatment
failure, sensitivity testing (of your H pylori strain) may be needed in the
event that your bug might be resistant to the antibiotic they prescribed, if
they did.
So, persistant duodenitis may be due to H pylori that they failed to
diagnose due to false negatives on the testing, or if they did indeed find H
pylori, they may be treating it with the wrong antibiotic due to antibiotic
resistance.
