notation for letter - suggestions please

| I received the following note from the OM. I've never read anything like
this on a letter, so would appreciate any suggestions for wording, and
placement on the letter. TIA.
"We recently had a chart audit, and one of the things that we need is a note
on every letter stating that the letter has been reviewed by physicians
prior to printing or something like that. The doctors review the dictation,
and then I print it on letterhead and sign it for them with my initials
after their name."

| My docs have "Dictated and authenticated by:" followed by their name on
the line below, their initials, and my initials. Would that do the
trick?

One option would be to include a disclaimer or something similar at the
very bottom of the correspondence (that is, a couple of lines after the
initials and CCs and all that good stuff). I wouldn't think it was
essential though as the signatures are initialed by the OM.

It's more of a "claimer" than a "disclaimer" isn't it? :-)
Anyway, yes, I think at the bottom after their sig is the logical place...
but I am wondering about the wording, since I've never seen something like
it before. Thanks.
