Author H&P sample report formats

I'm a student with MTEC and am having a tough time with H&P reports. I
can't seem to find enough samples of those types of reports and was
wondering if anyone can direct me to a site that has them.

The best thing I ever found was HPI's book H&P: A Nonphysician's
Guide, available at www.hpisum.com. The latest version is awesome.
