In listing the hemodynamic data from a cardiac cath, the doc has
dictated aortic pressures pre and post left ventriculogram; what he
normally gives are the left ventricular pressures. Does anyone know if
they do pre and post LV-gram aortic pressures or whether he may have
dictated this in error? (He never does give left ventricular pressures
in this particular dictation, which makes me wonder even more...)

What's the patient's diagnosis? There are certainly some cases when aortic
pressure pre and post stress (LVgram) would be of interest.

Indications were known CAD, previous CABG in 1995, and several weeks of
progressive angina and abnormal stress echocardiogram.

These were the conclusions the doc arrived at:

1. Normal left ventricular size and vigorous systolic function,
estimated LVEF 75-80%.
2. No regional wall motion abnormalities.
3. Mild elevation of left ventricular end-diastolic pressure post A wave.
4. Severe native coronary artery disease:
a. Complete occlusion of the proximal LAD.
b. Complete occlusion of the proximal dominant right coronary artery
just beyond its ostium.
5. Bypass graft patency as follows:
a. Patent left internal mammary artery to the mid LAD with a concentric
40% stenosis at the surgical anastomosis.
b. Patent sequential saphenous vein bypass graft to first and second
diagonal branch vessels.
c. Occluded saphenous vein graft to the distal right coronary artery.
6. Collaterals from the left coronary artery and diagonal branch
vessels fill the distal right coronary artery vessels.

I think I'll just leave it as dictated and flag it. He may very well
mean what he said, as he's usually very meticulous in his dictation, but
it's different from his normal format.

He might have been checking to see if there was LVOT obstruction as a result
of the hyperdynamic LV, although he probably should have given you LV
pressure as well in that case. OTOH, maybe he just wanted to show there
was/wasn't decreased coronary filling pressure post stress due to outflow
obstruction, given the degree of coronary disease. I would go along with
flagging as you decided though--it is not his normal dictation, and you
aren't a mind reader.
