Nevermind that London and Amsterdam are both immense metropolitan centres,
are home to some of the largest mental health "treatment" facilities
anywhere, and have street scenes big enough that the mentally-ill who fall
through the official cracks (as well as those who have nowhere to go when
released from hospital) can survive there with much less effort than they
could do in a Welsh village.
